Announcements

	As of June 1st we will no longer accept Medicare. We no longer accept GHI, Medicaid, HUMANA or TRICARE plans.
	Please bring the following items with you to all appointments: 1. Your drivers license and an up to date insurance card 2. All medication and vitamin/herb bottles that you are currently taking. 3. Information about your prior physician for us to obtain a copy of your prior medical records. 4. Please have your co-pay and any unpaid balances available for payment.
	For those who do not wish to use insurance, we charge only $85 for up to a 20 - minute office visit. Lab work may be billed to your insurance company.
